Take Action Program for Weight Management is a highly successful 12-week program for people who want to lose weight and keep it off. The program combines a personalized fitness profile with a body composition measurement, personal exercise training session, nutrition consultation with a resting metabolism measurement and an integrative health and wellness coaching session. Twelve weekly, on demand (recorded) sessions cover topics including nutrition, fitness, motivation and overall wellness. A comprehensive Take Action manual is provided for ongoing reference. Additionally, Allina Health employees may receive unlimited wellness coaching to support their journey.
Additional Information:
** In order for Allina Health employees to receive the program for FREE: a Program Payroll Agreement must be signed prior to class. All seven appointments must be attended, and all 12 online classes must be completed.

The program has seven required appointments. The fitness-related appointments are done in-person at LiveWell Fitness Center, on the Abbott Northwestern Hospital campus. Virtual appointments are available for the nutrition and coaching appointments. The four intake appointments must be completed prior to viewing your first weekly class. After registering for the course, please contact Angel Teske to schedule the appointments 612-863-5178.
The four intake appointments to be completed before beginning the class are listed below. These are 1:1 in-person or virtual sessions and are included in the package price.

1. Fitness Profile (90 min)
2. Personal Training (60 min)
3. Nutrition consultation with metabolism test (60 min)
4. Wellness Coaching (60 min)
Follow Up Appointments:
5. Mid-point Body Composition (30 min)
6. Mid-point Coaching (30 min.)
6. Final Body Composition (45 min)
